Our clients are looking for Care Managers to join their busy and thriving teams.

Hospital discharge experience essential because candidates must be able to hit the ground running. 

The role comprises: 

Assessment of needs, reviews, care planning and discharge planning for adults 18 years +

Working in a multi-disciplinary setting

Liaising with other professionals, including OT, physios, doctors, police etc.

Sign-posting to housing department

Safeguarding vulnerable adults 

In addition, candidates must have a recognised professional qualification in social work, e.g. DipSW, CQSW, degree, with full GSCC registration. An enhanced CRB check less than 12 months old is also essential. 

This is a temporary position. Candidates must be able to demonstrate the right to work in the UK without restriction in order to apply.
